The technique I use is called the Transoral Endoscopic Thyroidectomy Vestibular Approach (TOETVA). In simple terms, I make a few small incisions inside the lower lip, insert a camera and fine instruments, and reach the thyroid gland from underneath. Since the cuts are hidden inside the mouth, there’s no visible scar on the outside. This option is suitable for carefully selected patients, including those with: Small, non-cancerous nodules (benign growths) Early-stage thyroid cancer (when the tumor is small and hasn’t spread to lymph nodes) Overactive thyroid conditions such as Graves’ disease, when surgery is required Not every patient is a candidate, but for the right cases, it offers an excellent alternative. How the Surgery Is Done The operation is performed under general anesthesia, so the patient is fully asleep and comfortable. Step 1: Small incisions are made inside the mouth, between the lower lip and gums. Step 2: A high-definition camera and special instruments are introduced. Step 3: With the help of advanced visualization—and sometimes robotic assistance—the thyroid is carefully separated from nearby delicate structures, such as the parathyroid glands (which control calcium) and the voice nerve (recurrent laryngeal nerve). Step 4: The gland is removed through one of the same small hidden cuts. Robotic systems add even greater precision by providing magnified 3D vision and fine instrument control, allowing movements beyond what a human wrist can do.
